Use this form to report issues such as defective or incorrect products, or incomplete orders.

✍️ How should you fill it out?

🔢 Step 1

Complete all fields with the required information:

⚠️ Important: The information requested may vary depending on the supplier ID you enter, as each supplier has specific requirements for handling warranties.

⚠️ Key requirement: You must have an image of the packaging and its label.

📸 If the customer does not have this image, please indicate this in the reason for the claim and attach another image so that we can continue with the process.


📌 Step 2

Select the reason for the claim:

  • Defective or broken product
  • Product shortage
  • Incorrect product

📤 Step 3

Once all fields are completed, click on “Submit” to send the form.

The case will be referred to the suppliers area and will remain pending review.


📞 Step 4

The supplier department will analyze the case and, once resolved, the warranty team will contact you via WhatsApp to inform you of the result.

Please note that the resolution may take up to 7 (seven) business days after you upload the form.
